Printed on parchment. In the right-hand lower corner an unidentified woodcut coat of arms: per fess, in chief a unicorn salient, and in base six flames inverted, not as BMC. Sheppard notes that these arms are not those of Peraudi, as given in Giovanni Palazzi, Fasti cardinalium omnium sanctae Romanae ecclesiae, II (Venice, 1701). Guarded on a sheet of parchment.
Binding: Bound in a modern guard-book of fragments, assembled in 1954.
Size of leaf: 150 × 200 mm.

Provenance: Johannes de Vorda, Benedictine monk (fl. 1488); inscription dated 21 Apr.: ‘Dominus Johannes de Vorda monachus ordinis sancti Benedicti'. Date of acquisition unknown.
